Sex supplements: Do these things actually work?

However, nobody monitors sexual supplements, which is what makes them scary. The FDA warns that these supplements may contain prescription drug ingredients, controlled substances, as well as untested and unstudied pharmaceutically active ingredients. It issues extra caution concerning sexual supplements (they even use an exclamation point in their official warning):

“These deceptive products can harm you! Hidden ingredients are increasingly becoming a problem in products promoted for sexual enhancement.”

As can probably be expected, little research has been done on sexual supplements. Of the many pills and potions being touted as sex enhancers, only a handful have been studied in any capacity.

Ginseng

Ginseng is the most common ingredient included in the top-selling sexual supplements. In addition to being used as an aphrodisiac, ginseng is theorized to improve sexual function by inducing relaxation of the smooth muscles of the corpus cavernosum via the nitric oxide pathway. However, the side effects of ginseng include headache, upset stomach, constipation, lower blood sugar, and more. These adverse effects don’t bode well for the bedroom.

Fenugreek

Fenugreek is found in one-third of the top-selling sexual supplements created for men and is likely safe. Also known as “methi,” fenugreek is believed to improve hormonal regulation, with possible effects on male sexual health. In one study, researchers found that its use was associated with improved sexual arousal and orgasm, with no adverse effects.

L-arginine

L-arginine is the amino acid used to make nitric oxide, a molecule that facilitates the flow of blood to the penis during an erection, and is the most common amino acid found in sexual supplements. It’s unclear, however, whether a pill form of L-arginine helps with sexual stimulation. Moreover, people with heart disease shouldn’t take L-arginine.

Yohimbe

Yohimbe is an evergreen tree found in Western Africa. Its bark is used to make extracts, tablets, and capsules, which are used to treat erectile dysfunction. Yohimbine hydrochloride is available as a prescription medication in the United States. Adverse events are rare, but its most common side effects include headache, sweating, agitation, hypertension, and insomnia. Yohimbine is contraindicated in patients taking tricyclic antidepressants, antihypertensives, and central nervous system stimulants.

Dehydroepiandrosterone (DHEA)

DHEA is a natural steroid prohormone in the body that declines with age. Some research suggests that DHEA increases libido in women and helps erectile dysfunction in men. Other research indicates that while DHEA supplementation appear to be safe, it fails to budge hormone levels.

Tribulus

Tribulus is an invasive plant species in North America and goes by the name bindii, goat’s head, or devil weed. Although it has boosted sexual activity in animal models, no effects have been demonstrated in humans. Moreover, there have been two reported cases of tribulus-induced severe liver and kidney toxicity following high doses in young men.

Horny goat weed

Although horny goat weed (ie, epimedium) has not been shown to boost sexual activity in humans, it is generally safe for use in its unadulterated form, with only mild adverse effects like increased heart rate and hypomania. In other words, horny goat weed is likely ineffective, despite its suspected action as a phosphodiesterase inhibitor and, of course, its promising name.

Zinc

Although safe, zinc, which is also commonly found in many sexual supplements, is unlikely to boost sexual function. Moreover, zinc deficiency is rare, so most people don’t need this nutrient supplemented.

Maca

According to animal models, maca use was associated with a boost in sexual behavior (muskrat love?). But it has no demonstrable sexual effect in humans. For the most part, the vegetable maca is associated with only uncommon adverse effects, such as mildly elevated liver enzyme and blood pressure levels.

Ginkgo biloba

Ginkgo biloba is advertised for all kinds of health benefits, including sexual. However, it has no proven beneficial effect on sexual function. Plus, it can cause headache and seizures, and interfere with the blood thinner warfarin, significantly increasing an individual’s risk of a bleeding adverse event.

Ultimately, other than their placebo effect, many of the supplements sold to boost sexual function are a waste of money. Moreover, these products can contain dubious or dangerous ingredients. Besides prescription medications, the only other proven ways to improve sexual dysfunction are lifestyle changes including diet, exercise, and smoking cessation. In addition, psychological causes of erectile dysfunction—due to anxiety, depression, guilt, stress, or relationship issues—may be improved with counseling.

If you have a low sex drive, can’t get off, or you have fertility issues, you’re not alone. In fact, you’re far from alone! About 40 to 70 percent of men experience some kind of sexual dysfunction in their lifetime, according to research from the Wake Forest Baptist Medical Center.

Problem is, when it comes to your penis, it’s a sensitive subject. Men are far less inclined to go to a doctor in the first place, let alone disclose their medical problems. So, many turn to over-the-counter supplements rather than go through the perceived embarrassment of their sexual faults and receive a prescription. That’s not to say supplements aren’t helpful in providing optimal health (even for your Johnson); but there are so many formulations, and little regulation over standards and appropriate dosages that it can be easy to do your body a disservice.

Before you begin taking any supplement, speak with your doctor for appropriate dosage, possible side effects to medications you’re currently taking, and advice on what might be best for you.

That said, here’s a guide of supplements either to be used alone or in tandem with others to treat common men’s sexual health issues. These options have been backed by research and/or given a nod from Wake Forest Baptist Medical Center’s 2016 review, which dissected all scientific evidence behind the effectiveness of common sex supplements’ ingredients and safety. 1. DHEA

Best usage: Enhancing sex drive, minimizing erectile dysfunction

DHEA is a hormone produced by the adrenal glands, which your body then uses to produce sex hormones, according to research from the Wake Forest Baptist Medical Center. Daily supplements may benefit people with decreased libido, improve sexual function, and minimize erectile dysfunction in men, per the Mayo Clinic. And, while there’s good scientific evidence for DHEA’s ability to bolster your sex life, studies have flip-flopped in terms of consistency; more data needs to be done to suggest the full range of its benefits and side effects.

2. Fenugreek

Best usage: Intensifying sexual arousal and orgasms, reducing recovery time

You’ll find this herbal extract in about a third of the top-selling men’s health supplements, per Wake Forest Baptist Medical Center. One study published in the journal Phytotherapy Research found men who took the supplement experienced an 82 percent increase in libido (more intense arousal) after six weeks. What’s more, 63 percent of the men taking fenugreek enjoyed a boost in sexual performance (stronger orgasms), 67 percent said the supp enhanced their sexual recovery time, and even promoted greater muscle strength, overall energy, and well-being. No participants reported negative side effects; plus, other studies back it up.

3. Ginseng

Best usage: Bolster overall sexual health, including proper penile function, libido, performance, sperm quality

Ginseng is a super power herb that’s long been touted as an aphrodisiac, treatment for erectile dysfunction, and overall sex enhancer in traditional Chinese medical practice; it’s also the most common ingredient in men’s sexual health supplements, according to Wake Forest Baptist Medical Center. An animal study published in British Pharmacological Society discovered ginseng has a favorable effect on libido and sexual performance, which crosses over into human models, too. Ginseng improved the sperm quality and count of healthy men, as well those with fertility issues. In another study, 45 men with moderate to severe erectile dysfunction had marked improvements with regards to performance and sexual satisfaction after supplementing three times a day with 900mg Korean red ginseng for 8 weeks. And, yet another study, published in the Journal of Urology, discovered ginseng helped men with erectile dysfunction enjoy better performance—greater rigidity, penetration, and maintenance of erections—after taking 1,000mg of Korean red ginseng three times a day for 12 weeks.

4. L-citrulline

Best usage: Combat erectile dysfunction, boost sexual satisfaction

L-arginine may be the most common amino acid present in men’s health supplements, per Wake Forest Baptist Medical Center, but it’s metabolized too quickly for there to be any real benefit, according to research published in Urology. That’s why L-citrulline, which your body converts to L-arginine, is a better choice. In a small study, done on men with mild erectile dysfunction, L-citrulline supps (1.5g per day for one month) amplified 50 percent of participants’ erection hardness scores from 3 (mild ED) to 4 (normal erectile function). Men also reported having more sex and were very satisfied with the results. Overall, l-citrulline has been proven safe and psychologically well accepted by patients with ED, researchers say.

5. Maca

Best usage: Stronger sex drive, sperm function

More studies are warranted, but a systematic review of research across 17 different databases found evidence for the effectiveness of maca in the improvement of sexual function. One in particular, published in Andrologia, found 40 percent of men who took maca plant supplements for three months experienced a stronger sex drive, as well as improved sperm formation and movement.

6. Yohimbine

Best usage: Minimize erectile dysfunction

If you’re feeling deflated in more ways than one, Yohimbine—the bark of an evergreen tree native to Africa—can help by widening blood vessels and lowering blood pressure, increasing the health of your sexual organs, yielding firmer erections, and combating erectile dysfunction, according to a meta-analysis published in Wiley. Researchers site a variety of studies in which men were given 18mg of Yohimbine per day to treat or minimize ED; in most cases the supplement didn’t cause significant non-intended side effects (though in some instances men experienced anxiety and headaches in higher doses, Wake Forest Baptist Medical Center notes). In two separate studies, 34-46 percent of men had a positive outcome.

7. Vitamin E

Best usage: Healthier sperm, fertility

For stronger swimmers, vitamin E supps can help protect and foster better-quality sperm, according to research published in the International Journal of General Medicine. Nearly 700 infertile men took 200 μg (micrograms) with 400 IU (units) of vitamin E every day for at least 100 days. Over 50 percent of the men had improvements in sperm motility and form, and roughly 11 percent of men were able to get their partner pregnant.

Since dehydroepiandrosterone (DHEA) was discovered in 1934, scientists have been probing its metabolism, endocrinologists have been studying its functions, doctors have been debating its uses and abuses, and the supplement industry has been touting its virtues. It’s a confusing mix of science, speculation, and commerce. What is the status of this controversial chemical?

What is it?

DHEA is a steroid hormone. Like cortisol and the sex hormones, it’s produced by the body, with cholesterol as its main building block. Most of that production occurs in the adrenal glands, small structures that sit on top of the kidneys at the rear of the abdomen. Lesser amounts are also manufactured in the intestinal tract and brain. Most of the DHEA in the bloodstream circulates in the form of a derivative, DHEA sulfate (DHEAS). The intestines also convert DHEA supplements into DHEAS before the hormone enters the blood. Since DHEA and DHEAS are so closely related and can be converted into each other by the body, it’s reasonable to consider them together as DHEA.

Vitamin D and DHEA Help Men Avoid Frailty: Study

With age, frailty—defined as a gradual loss of energy, strength and physical ability—leads to a loss of mobility, falls and even death, experts know.

Now, new research suggests that men with sufficient Vitamin D levels are more apt to avoid frailty. In older men, over 70, having enough male hormones may also help.

So vitamin D is not just good for bones, says Agnieszka Swiecicka, MD, a clinical research fellow at the University of Manchester in Manchester, U.K. She presented the findings Sunday, Apr. 2 at ENDO 2017, the annual meeting of the Endocrine Society, in Orlando.1

She evaluated more than 3,000 men, ages 40 to 79, over a four-year period. She assessed their hormone levels, vitamin D levels and their degree of frailty using standard measures. She looked at low muscle mass, exhaustion, how fast they walked and how much they exercised.

Vitamin D—Frailty Link

The men who had higher vitamin D levels at the study start were less likely to become frail later. With each unit increase in vitamin D levels, she says, the risk of frailty decreased. “Vitamin D were associated with a reduction of frailty in both frailty models ,” Dr. Swiecicka says.

“Vitamin D, besides maintaining bone health, regulates muscle function, and low vitamin D levels are linked to lower muscle mass and strength,” she says. In older men, over 70, those with higher levels of the male hormone DHEA-S, were also less frail, she says. For those 70-plus, higher DHEA levels reduced frailty by about 43%, she says. She speculates that DHEA-S may have a direct effect on the muscle. Recently, experts say it may also help the brain and the immune system.

Dr. Swiecicka found that those who became more frail over the follow up also had lower body mass indexes and were older, on average age 61. They were also more likely to have diabetes. While 5% of those who didn’t become more frail over the follow up had diabetes, 8% of those who deteriorated did.

Previous Vitamin D-Frailty Research

In a previous study, Australian researchers evaluated more than 4,000 older men, ages 70 to 88. They found that vitamin D status predicted independently who would become frail.

Those who had the lowest vitamin D levels, compared with those with the highest, were also about 20% more likely to die over a nine-year follow up. The risk of death was not solely dependent on frailty status, however. 2

A Dietitian Speaks

“Vitamin D intake is one that is challenge for most adults,” says Connie Diekman, RD, director of university nutrition at Washington University in St. Louis, who reviewed the findings. Yet, she says, ”the impact on bone health, immune function and other aspects of metabolism can be significant. As we age our ability to convert the inactive vitamin D in the skin declines, and often sun exposure decreases as well so that vitamin D deficiency in adults is common.”

“The current level of understanding about adequate vitamin D intake and the roles of vitamin D in health are evolving, but this study provides one more consideration for why we need to monitor vitamin D status to prevent some of the maladies of aging.”

How Much is Enough?

According to the Institute of Medicine, adults 51 to 70 should aim for 600 International Units (IUs) a day; those over 70, 800 IUs. (The upper tolerable limit is 4,000 IUs for children 9 years and above and adults.) However, your doctor may suggest a different amount.3

“Everyone is at risk of vitamin D deficiency, including older men,” says Michael Holick, MD, PhD, professor of medicine, physiology and biophysics at Boston University Medical Center, and a long-time vitamin D researcher. “I recommend to my older male patients the same as I recommend for all my patients. They should be on at least 3,000 IUs of vitamin D a day. I am on 4,000 IUs a day which is perfectly fine.” Your doctor can order a blood test to evaluate your levels, and base advice on the results.

Among the good sources of vitamin D from food is salmon, which has about 450 IUs per 3-oz serving, and swordfish, with more than 560 IUs in the same size serving. Many people rely on a vitamin D supplement to get enough.3

DHEA Past 70 and Frailty

Overall, having higher levels of DHEA, a hormone made in the adrenal gland and the brain, was linked with less frailty. However, after Dr. Swiecicka adjusted for age, she found that higher DHEA levels were linked with a lower risk of worsening frailty in men older than 70, but not in younger men.

In those over 70 years old, higher levels of DHEA reduced frailty risk by 43%.

More About DHEA

DHEA triggers the production of both androgens and estrogens, the male and female sex hormones. Levels of DHEA produced by the body begin to decline after age 30, experts know. 4 Due to potential risks of prostate, breast and ovarian cancers, DHEA supplement use is not suggested without guidance from your doctor. 4

Doses vary depending on the condition being treated, according to Mayo Clinic. For instance, doses of 50 to 200 milligrams of DHEA have been taken by mouth every day for up to two years to improve bone density. However, research is evolving. 4

Side Effects of DHEA Supplements

Some of the side effects of supplementing with DHEA, although minor, are related to mega-dosing in most cases (500mg+ / day). What is DHEA and What Does It Do?

DHEA stands for dehydroepiandrosterone and it is a steroid hormone. When DHEA is created by the body, it is converted into testosterone and a variety of sex hormones. Though DHEA is naturally produced by the body by the adrenal gland, a DHEA supplement can be taken in supplement form. It is often taken to increase testosterone and estrogen production, which in turn can boost a range of body functions and achieve health benefits.

Our DHEA creams are one of the most popular supplements for those who are wanting to boost their testosterone or estrogen levels, despite not everyone understanding what it does and the benefits of using it over hormone boosting methods. Whether we’re looking at men or women, DHEA production peaks in your late 20s and then slowly decreases with age. This is why, as we age, a lot of people choose to boost their DHEA.

What Does DHEA Do?

Scientists don’t know everything that DHEA does, but they do know that it converts into a range of male and female sex hormones; this includes testosterone and estrogen. This is all done naturally by the body, but additional DHEA can be taken in supplement form. DHEA helps to produce hormones, which means more hormones can be produced when DHEA supplements are taken.

DHEA encourages the body to produce more estrogen and testosterone, both of which are the main female and male sex hormones respectively. A number of health benefits can be fought with a high level of estrogen and testosterone, which is why a lot of people choose to take it. DHEA can be taken in tablet form or as a topical cream, depending on how it is being used.

The Benefits of DHEA

There are a number of benefits that come with using DHEA, which is why it’s such a popular supplement. The most well known is that DHEA boosts testosterone and encourages the body to produce higher levels of testosterone, something that slows with age. As they get older, a lot of men use DHEA to boost testosterone. This is because increased testosterone brings with it a number of advantages. For example, testosterone:

  • Improves sexual function, sex drive and sperm production.
  • Increases energy and athletic performance.
  • Increases muscle mass and bone density, as well as decreasing fat mass and the chances of developing obesity.
  • Improves the appearance of skin and skin health.
  • Boosts mood, memory and brain function.
  • Improves immune function and overall health.

As you can see, there are a lot of benefits that come with taking DHEA to boost testosterone. However, it also boosts estrogen levels in women and this is often why it is referred to as an anti-aging therapy. A large number of women struggle with the signs and effects of aging, but a number of these can be fought with high levels of estrogen. DHEA has been known to slow the aging process, improve cognitive function, improve vaginal dryness and even help depression. So, whilst the main benefit of DHEA is to boost testosterone and estrogen levels, this can lead to a number of other benefits. Are There Are Side Effects of Taking DHEA?

Taking DHEA is a relatively new treatment, so there isn’t a great deal of scientific evidence on how it works. This also means there isn’t a great deal of evidence on the extent of the possible side effects. However, that’s not to say that there isn’t any. As DHEA is a hormone supplement, there can be side effects to using too much or using it incorrectly. This could include high blood pressure, boosts in depression, fatigue, hair loss and stomach upsets. However, when used responsibly, DHEA can be extremely effective. As with all supplements, the best results are seen when DHEA is used correctly and with caution.
